You must therefore sign this form before you are allowed to serve alcohol. It is essential that if you do not understand any of these instructions or require further information, that you do not sign this declaration and speak to your Manager.Breaking the law can result in heavy fines and the loss of our licence. In the event of authorities discovering that you have served alcohol to someone under the age of 18 or someone who is drunk, YOU could be issued with an £80 FIXED PENALTY NOTICE, which could be your personal responsibility to pay. A conviction for any of the matters listed below not only leaves you with a criminal record, but may prevent you from becoming a personal licence holder in the future. In addition, if you do not comply with these rules, disciplinary action may be taken against you, which could result in the loss of your job. Please tick each declaration *Do not sell alcohol to anybody under the age of 18. If you are in doubt about their age, simply do not sell alcohol to them. If the customer looks under 25 , you must ask them for identification. The only acceptable forms of identification are a valid passport , or a UK Photo Driving Licence.Do not sell alcohol to anybody you suspect to be drunk. In addition, you should not sell to anyone whom you believe to be purchasing alcohol on behalf of an individual who is drunk.Do not serve alcohol outside the hours listed on the premises licence. You must also make sure that drinking up is complete within 30 minutes. You must be aware of any conditions on the licence and abide by them.Always ensure that you serve the correct measures. It is an offence to give under & over measures.Do not knowingly deliver or knowingly allow somebody else to deliver alcohol sold on the premises to a person under 18 years of age.I can confirm that I have received training in all the above matters (Licensing Act 2003) and have read and understand the details of these instructions. FINE UP TO £1000A person commits an offence if they sell alcohol to an individual under 18. FINE UP TO £5000It is an offence for a member of staff to knowingly allow under 18's to consume alcohol. FINE UP TO £5000Police and Trading Standards Officers are empowered to send under 18's into licenced premises to attempt to buy alcohol (subject to certain guidelines being complied with). It is not an offence to suggest entrapment. Because it is extremely difficult to assess age, it is Breeze's policy to challenge any person attempting to buy alcohol you believe is 25 or under. NOT 18 or under.Defence:That you believed the individual was 18 or over AND you had taken reasonable steps to establish the individual's age OR nobody could reasonably suspect from the individual's appearance that he/she was under 18. ALTHOUGH THE LAW REQUIRES YOU TO BE SATISFIED THE INDIVIDUAL IS 18 OR OVER, IT IS COMPANY POLICY THAT YOU MUST BELIEVE THE INDIVIDUAL TO BE 25 OR OVER OR YOU MUST ASK FOR IDENTIFICATION.Reasonable steps:A person would be said to have taken reasonable steps if: He/She sought proof of age & believed proof was genuine AND the evidence of proof of age provided would convince a reasonable person.Proof Of AgeA photographic Driving Licence or A Passport are the only documents that are valid.
